It is required that you complete the forms provided and return them to Property Management. The information provided will enable our staff to provide the best service possible to you and your employees.

To make your transition as easy as possible, we would like to obtain some information from you. The following forms should be completed and returned to our office at your earliest convenience:

Tenant Information Form
This form will be used by Property Management to verify employee authorization and contacts for after-hours emergencies, day to day operations, and billing questions.

Key Request Form
Your suite is keyed to the building master system, and keys are issued upon initial occupancy. Additional keys can be purchased throughout your occupancy for a small fee by using this same form.
This form plays an important part in tracking all persons who have access to your suite and maintaining the building security. Please be certain to alert Property Management and collect suite keys if someone leaves your employ or is terminated. Any keys left uncollected may leave your suite vulnerable.

Company Vehicle Information Form
Please indicate on the enclosed form the information related to any company vehicle that will be parked on site. This should include your business name and vehicle information (year, make, color, and model).

Tenant and Vendor Insurance Certificates (See Samples)
A sample tenant insurance certificate and sample vendor insurance certificate are provided for your reference. Please supply insurance certificates following these examples.
